Step 1 of the workflow — identify the subject property and record its county characteristics.
Searches the NYS assessment roll and fills what it publishes. It does not carry style, living area, lot area or year built — those still come off the Land Records Viewer. Optional: you can fill the whole form by hand.
Enter the address and parcel exactly as they appear on the county record. Look them up in the Nassau County Land Records Viewer.
These drive the comparable-sales similarity score in a later step, so record what the county has on file — not what you believe is correct. Note any disagreement below. The state roll does not publish any of these four for Nassau, so they always come from the Land Records Viewer, never from the lookup.
Condition issues, suspected record errors, renovations — anything that may matter as evidence.
Use this when the county has the property itself wrong — overstated size, condition, features, or use. That is a separate claim from an over-assessment, with its own evidence.
